Welcome to the Mansfield Minor Baseball Association (MMBA) Online Registration Form for 2023! PLEASE COMPLETE ONE FORM PER PLAYER. PLAYERS NEW TO MMBA MUST EMAIL A COPY OF THEIR BIRTH CERTIFICATE TO [email protected] Due to previous uncertainties around Covid-19 and varying registration numbers for certain Divisions, MMBA will maintain a 2-stage registration for the 2023 Season: STAGE 1 - Please register using the online form below. This will establish a priority list for assigning players to a team. STAGE 2 - One time full season payment by e-transfer to [email protected] once the season is confirmed (April, 2023). HOUSELEAGUE - Approximate fees for a full season will range from $105.00 (6U) to $280.00 (18U). REP/SELECT - IN ADDITION to the $200 paid to date by Rep and Select families, each player will need to pay the Houseleague registration fee for the appropriate Division (i.e., 11U, 13U, 15U, etc.) which will be due in Stage 2, early April, 2023. *Please note: The initial total of $200 is used for Tournament registration, uniforms and other expenses that Rep and Select Teams have due PRIOR to Season start. The Houseleague fee to be paid per Rep and Select players will be allocated to remaining Season expenses such as diamond time, umpire fees, insurance, etc. Rep and Select Team Managers/Coaches are provided a Budget Summary detailing the expenses.     By completing this form, you are registering with both MMBA and Baseball Ontario. 

    Baseball Ontario appoints MMBA to collect fees on its behalf, included in the registration fee collected through this process. No additional monies is charged to the parent registering locally. Baseball Ontario invoices MMBA for fees collected.

    On behalf of myself and the player being registered, I recognize that baseball entails serious risks. Consequently, except as noted below, I and the player being registered relinquish all rights to a claim of any kind, including the right to a claim for bodily and material damages, regardless of the cause, against Baseball Ontario, its member associations, including MMBA  and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors, even if such damages result from negligence of Baseball Ontario, its member associations, including MMBA   and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors.

    Without restricting the generality of the preceding, I and the player being registered also relinquish the right to any claim against Baseball Ontario, its member associations, including MMBA  and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors resulting from a decision on their part, regardless of the nature of this decision.

    This release of Baseball Ontario, MMBA and the other persons noted above does not preclude myself or the player being registered from making a claim under any sports accident coverage provided by Baseball Ontario and/or MMBA to their players.  An overview of the current sports accident coverage can be provided by Baseball Ontario.
  3. Refund Policy:

    Any player choosing to not participate shall have until the first regular season game played by his or her team to earn a full refund.

    After the first game no refund will be due except in special circumstances such as season ending injury/illness.

    Should circumstances cause a team or league to be cancelled, refunds will be issued as determined by the association treasurer.
  4. Rowan’s Law:

    Under Rowan's Law, before any player can be registered with the local association and Baseball Ontario, the player, and the parent or legal guardian of the player if the player is under 18 years of age, must review one of the Concussion Education Resources provided by the Province of Ontario and must review the Player Code of Conduct.     Rowan's Law Acknowledgement:

    I hereby confirm that the player being registered to participate in baseball activities with the local association and Baseball Ontario, and the parent or legal guardian of the player being registered if the player is under 18 years of age, have reviewed one of the concussion education resources provided by the Province of Ontario as referenced above and have reviewed the parent and player code of conduct as referenced above. I / we further acknowledge understanding the nature and risk of concussion and head injury to athletes, including the risks of continuing to play after a concussion or head injury is suspected.
  5. Photo Consent:

    I authorize MMBA and/or its staff, associates, assistants, or subcontractors to use photograph(s), video and other media image provided for any MMBA  promotional, educational or other pertinent uses. I also agree to MMBA  using any images of the named minor(s) gathered by photographers, and or videographers supplying images to MMBA.

    These images may include, but are not exclusive to, club, coach, athlete and/or parent, and/or volunteer submissions by various member associations of MMBA.

    I authorize MMBA to permit the use and display of photographs and/or recordings of the named minors in any MMBA publication, multimedia production, including video and web usage, display, or advertisement.

    I agree that MMBA may use name, likeness, or information supplied by the undersigned.

    The undersigned releases and forever discharges MMBA and its officers and employees from any and all claims and demands arising out of or in connection with the use of said photographs / recordings, including but not limited to, any claims for invasion of privacy or defamation.

  7. Privacy Policy:

    MMBA collects and uses Personal Information solely for the purpose of conducting its operations and offering its programs and services to players, coaches, umpires and member associations. MMBA hereby asserts that Personal Information will only be used for such purposes and not sell, trade or barter Personal Information. 

    MMBA will not disclose Personal Information provided to it, unless such disclosure is required in order to provide a participant or member organization with our services.

    MMBA is governed by Baseball Ontario and I agree also to the Baseball Ontario Privacy Policy
